Wholesale Information Request In order to release our wholesale pricelist, those legal types tell us we first need the following documents from you. California Customers Wholesale Information Request Form California Resale Certificate All other states Wholesale Information Request Form A copy of your valid state resale license or seller's permit. Opening Orders There is a $300 minimum for all opening orders. Prepayment is required for all opening orders. In addition to the above resale information, prior to or upon placing your first order we will need the following documents signed and on file: Terms and Conditions Credit Card Authorization Form Placing Orders Kalola accepts orders via phone, fax and email. Feel free to use your own purchase order or our convenient order form found below. Phone orders require a signed order confirmation before going to production. Kalola Order Form Minimums The only minimums we require are the dollar amount on your opening order. Other than that, you can expect no minimums on reorders and no quantity minimums either. Only you know best what your customers want and what your inventory needs are! Lead-Time All of our products are made to order. While often sooner, you can generally expect your shipment to leave our factory 3-4 weeks from the date of order. Transit times vary based on final destination. Rush orders can be accommodated for an additional 10% plus applicable expedited shipping fees. Unless otherwise specified, Kalola ships via UPS ground FOB from Los Angeles, California.
